What to Bring

As a general rule of thumb, the more information you bring to your appointment the better it will help us give you a diagnosis, prognosis and the best recommendations available to you.

Items to remember to bring:

X-ray, MRI, CT, & Arthrogram Films and Reports

You will need to obtain any recent x-rays, MRIs, CTs, and Arthrogram films and reports that relate to the problem that will be evaluated at your visit. Please bring the films AND reports yourself, as it is often difficult for others to get us the x-rays and reports before your appointment.

Some offices or hospitals may give you a disc instead of printed films. Bringing this disc is fine because we are able to view your films and reports in our computer system.

Medical Records

Please obtain and bring with you a copy of your medical records.

Surgical Report

If you have had a previous surgery on your shoulder, elbow, or knee, we need to have the most recent operative report so that we know what type of surgery you have had. If your previous procedure was an arthroscopic surgery, please obtain copies of arthroscopic photographs from your surgeon for us to review.

Referral

If required by your insurance carrier, obtain a referral from your primary care physician and bring it with you.

Insurance Card

Bring your insurance card. We will need to make a copy of the card for your records.

Photo ID

Bring a photo ID (driver's license, passport, state-issued ID card, etc.).

Co-pay

  • You insurance may require a co-pay that must be paid at the time of your visit. Please bring some form of payment.
  • A financial policy form must be completed each calendar year (can be found in new patient packet)
  • You will need to complete a medical questionnaire for any of the following reasons:
    • If this is your first appointment
    • If it is an appointment for a new problem not previously evaluated in the office
    • You can download a copy of the medical questionnaire within the new patient packet here and complete it before your visit.

Workers Compensation

  • If this is a work-related injury:
  • Please bring a copy of the first report of your injury including your claim number.
  • Bring any x-rays or reports that relate to this injury.
    • If, for any reason, workers' compensation contests or denies your claim we can submit the charges to your health insurance only if you have followed their rules and have obtained a referral, if required.
    • If workers' compensation denies your claim and you have not provided us with your health insurance information or followed the guidelines of your policy, you are responsible for all charges.
